# Documents: extracting information without losing evidence

Turn files into reviewable data while preserving source, version and context.

## Extraction is not correct interpretation

Structured fields help but do not guarantee meaning. Amounts may be subtotals, deposits or balances; dates may indicate issue, validity or delivery. Define fields and supporting evidence. Keep source excerpts or references for review. Missing data should remain explicitly missing, never disguised as extraction through inference. Preserve the distinction between a value explicitly present in the file and a conclusion derived by a reviewer. Both can be useful, but they need different labels and evidence. A date copied from a document should not silently become a deadline if the source does not define it that way.

## Design the schema with the team

Schemas should serve operational decisions. Quote comparisons need conditions and exclusions; case reviews may need issuers and versions. Start with important fields and define formats, missing values and multiplicity. Do not flatten line items if quantities later require item-level verification.

## Validations that add value

Format and document correspondence checks detect errors without replacing content review. Flag inconsistent totals, duplicates, missing pages and later versions. Reviewers need extracted values beside evidence, with correction and reason tracking. Corrections inform schema or instruction improvements. Organize the review around fields with consequences for the next step. A reference used to join records may deserve a different check from free-text descriptive material. The reviewer should be able to mark a field as unresolved and explain why, rather than being forced to choose a value just to complete the form.

## From review to authorized action

Writing extracted data elsewhere adds consequences. Drafting differs from updating cases or issuing orders. Track reviewed fields and required authorization. Preserve review state after write failures. Original-document references support later checks. Value includes verifiable handling of incomplete or contradictory files, not just faster entry. The destination system may require a format or identifier that differs from the source document. Record any approved transformation so the team can explain how the final value was produced. If only part of an update succeeds, retain the successful operations and isolate the uncertain ones for recovery. This prevents a document-processing flow from restarting the whole action and creating duplicate records after a partial failure.

## A representative test set

Test readable documents, photos, varied formats, missing fields and versions. Review error types, not just aggregate accuracy: a misread identifier can matter differently from an abbreviated description.

## A hypothetical difficult document

The following case is illustrative and is not based on any real document. An invoice arrives photographed with a phone, slightly tilted, with a stamp over the total and a handwritten note in the margin changing a quantity.

A system that only looks for the total field might read the amount partly covered by the stamp and deliver it with apparent confidence. A well-designed journey treats that result as unreliable: it flags the field as low confidence, keeps the original image next to the extracted value and sends it to review instead of moving on. The handwritten note raises a question no extraction answers on its own: is it a valid correction or a personal annotation.

The reviewer sees the document, the proposed values and the warnings, and can correct with a click without hunting for the file elsewhere. Their correction is logged with a reason, such as illegible stamp or quantity edited by hand, and that reason feeds improvement: if stamps over totals are common, a different capture instruction may be in order. The point of the example is that doubt is not hidden. It is surfaced, resolved and learned from.

## Key takeaways

- Keep evidence beside extracted data.
- Separate reading, review and writing.
